Clinical Information
Relevant Clinical History and Physical Exam
63 years old male known Accelerated hypertension, diabetic presented with chest painassociated with shortness of breath. CAD-Non ST Elevation myocardial infarction.On Examination-PR-52/min, BP-150/70mmHg, RR-70/min, SPO2-98%, CVS-S1+,S2+, P/A-Soft BS+, RS-Bilateral air entry adequate,CNS-No focal neurological deficit. Investigations-Hb-12g/dl, Serumcreatinine-0.8mg/dl, RBS-125mg/dl.
Relevant Test Results Prior to Catheterization
ECG showed NSR, 2D Echo revealed fair LV function, EF-54%.
Relevant Catheterization Findings
Coronary angiogramrevealed double vessel disease with D1 Osteal 90% Stenosis, Ramus-osteoproximal 90% Stenosis.
Interventional Management
Procedural Step
Procedure performedthrough right radial artery LMCA engaged with 6Fr EBU 3.5 Guiding catheter, 0.014X180cmRun-through wire placed in LAD and another Run-through wire was placed in Ramus.Predilatation done with 2.0x12mm Balloon. RAMUS stenting was done with 2.75X18mm Onyx DES. Diagonaldilatation done with 2.0 x 25mm PREVIL Drug Eluting Balloon. Thefinal result was good with TIMI III Flow without any edge dissection andresidual stenosis. Patient was discharged in a stable status on 3rd postprocedure day without any CV Symptoms.
Case Summary
Drug eluting balloons are helpful in places whereBifurcation and Trifurcation branching points are seen in a coronary vessel. Sothat we can avoid main vessels pinching. DEB can reduce metal load atBifurcation and Trifurcation anatomy.
