Effect of Postconditioning in Patients with ST-elevation Acute Myocardial Infarction.

Background: Postconditioning (Pos-con), which is brief period of ischemia performed just at reperfusion, could reduce the infarct size in animal models. We aim to evaluate the impact of Pos-con in patients with ST-elevation acute myocardial infarction.
Methods: Fifty-four patients were randomly assigned to the control group (n = 27) or the Pos-con group (Pos-con group, n = 27). In Pos-con group, we applied 4 cycles of 30-second balloon inflation and 30-second balloon deflation immediately after thrombectomy.
Results: No clinical adverse event during the procedure occurred in either group. The peak creatine kinase (CK) level in Pos-con group were significantly lower than those in control group (3191.8 กพ 1470 vs. 4229.0 กพ 1872 IU/ml; p = 0.03). ST-segment resolution in Pos-con group showed significantly better than those in control group (Pos-con, 59.0 กพ 24.7%; control, 39.3 กพ 40.8%; p = 0.03). Patients in Pos-con group showed better myocardial blush grade compared with patients in control group.

Conclusion: Pos-con is a simple, safe and feasible strategy to reduce the infarct size for patients with ST-elevation acute myocardial infarction.
